Make a Claim


If you have paid up front for medical services (doctor visits, emergency hospital expenses, lab tests, x-rays, etc.), you will need to submit a claim for review for reimbursement (receive money back).
If you have receipts for prescription drugs, dental, vision, or paramedical (massage, physio, etc.) expenses please submit these to the DSU Extended Health & Dental Plan – information on how to submit this type of claim can be found here.

*If you were a student covered during the 2019-2020 school year, you must submit any claims provided they are less than 1 year old to Medavie Blue Cross™ within 90 days of your coverage terminating. if you do not submit your claims by the 90 day deadline, your claim will not be processed and no refund will be provided.
  • Ensure you complete as much of the form as you can to avoid delays in processing.  You must include your name, student ID number and contact information.
  • Include the invoice or receipt from your medical appointment with your claim form.
  • Include any medical notes, doctor notes, lab results or prescriptions to support your claim.
  • If mailing your claim, please keep copies of all of your documents.
